the web application creates a page for all the form submissions and this form is submitted to same  url each time there is a different title tag when the page is loaded each time does this mean that there are different page with the same url  will it effect my seo

the one way is to format your url  to a unique page using a rewriter rule if you if you continue loading to the same page then google will always index to that page
